Need a direct connection to a SIP softphone from HMP

Hello,

a) I have an app in which I directly connect to H323 softphones.

b) Now we need to replace the H323 softphones with SIP softphones. I need some advice in how to directly connect from my app to the softphone.  Please bear in mind that you can make a direct connection from one softphone to another softphone, without the need of a SIP server.
